Great way to describe care management! Confirmation of practice to include treating the whole person; creating teams of different disciplines to effectively care for our patients.

Content was great. Very good explanation of roles in the care coordination process in CO. Unfortunately answers to the Q &A part were hard to hear. Otherwise very informative information.
