Jami Falk is an Office of Primary Care (OPC) Nurse Leader with over 25 years in health care, 21 years of progressive leadership experience within the Veterans Health Administration, spanning critical care, mental health, and enterprise-level quality management, and primary care. For the past 15 years in ambulatory care, Ms. Falk has led the development of primary care nursing practice guidance and transformational coaching initiatives focused on population health, chronic disease management, and care coordination and transition management quality improvements. She has extensive expertise in advancing primary care team-based practices and system-wide quality improvement across facility, regional, and national levels. She is committed to improving care for underserved populations, expanding access to women’s health services, and advancing rural health through telehealth.
Ms. Falk provides executive oversight to ensure safe, efficient, and high-quality nursing practice, leading process improvement efforts through effective change management strategies. She holds a Bachelor of Science in Nursing from the University of Jamestown and a Master of Science in Strategic Leadership from the University of Mary. She is a Certified Nurse Manager and Leader (CNML) through the American Organization of Nurse Executives and actively contributes to professional organizations, including AAACN, ACHE, and ANA.
