Jose A. Colon goes by his nickname of Tony. He began his nursing career at the University of Puerto Rico. He joined the U.S. Navy in 1983 and retired in 2007 with 24 years of military service. He completed his undergraduate degree of nursing at Florida State University and a Master’s in Education at Old Dominion University. His passion for diabetes education started in 2004 while still on active duty and became a CDE in 2005. He currently works at the Captain James A. Lovell, Federal Health Care Center in North Chicago, IL (the first fully integrated VA/DOD facility in the Nation). He is the team leader for a Primary Care Diabetes Team. Tony provides diabetes education and management to patients via different modalities including individual face-to-face, Shared Medical Appointments, and Telehealth. He is also a Teleretinal Imager. Tony's strong believe in the importance of the nurses' role as educators in every aspect/setting, led him to work on this research project in order to help evaluate and implement educational opportunities for nurses.
