Tiffany Hoffman is the lead advanced registered nurse practitioner in the transitional care center at Tampa General Hospital in Tampa, Florida. She has worked as a nurse practitioner for over 25 years. Her career includes Medicare wellness visit nurse practitioner, as well as acute inpatient care, and ambulatory cardiology care experience. It was in ambulatory care where her role was expanded to see patients who had been discharged from the hospital within 24-48 hours. During this time, she began to understand the barriers and care navigation gaps that patients experience. At Tampa General Hospital, she and her team have managed the care transitions during multiple COVID surges, followed by the expansion to medically complex patients.
