Overview

Explore the relationships among health care utilization, neighborhood disadvantage, and food insecurity. Study findings are discussed within the context of the five key activities to facilitate integration of social care into care coordination and transition management practice: awareness, adjustment, assistance, alignment, and advocacy. Implications for future research are discussed.
