This session explores the design of a primary care practice that acknowledges that care does not just happen at the hospital or at a specific practice. It also examines how nursing can motivate, create, and make an impact for the patients in a complex primary care center. The presenters outline approaches to expand the role of nurses including intervisit interventions that focus on nursing support.
