Care Coordination and Transition Management (CCTM) Special Interest Group (SIG) Session: Assessment and Evaluation of Ambulatory Care Registered Nurse Needs Regarding Care Coordination and Transition Management

Ambulatory care is a rapidly growing field of nursing practice. As this specialty practice grows, there have been efforts to identify the desired role of the registered nurse in ambulatory care and link it to patient outcomes. In particular, it is ensuring patients and families receive the right care, at the right time by the right provider with an end result being improved patient outcomes. This session discusses a project to assist in the development of a care coordination and transition management best practice repository through the CCTM Special Interest Group (SIG).
