CCTM SIG - Leveraging AAACN Toolkits: Demonstrating RN Value through CCTM and NSI SIGs Collaboration to Enhance Patient-Centered Outcomes

In this presentation, we aim to share the collective efforts of the Nurse-Sensitive Indicator (NSI) and Care Coordination and Transition Management (CCTM) Special Interest Groups (SIGs) to highlight the AAACN resources to provide actionable steps to address the gaps in defining and measuring the RN practice in ambulatory care.
