Medicare Wellness Visits: A New Role for the Registered Nurse in the Ambulatory Care Setting: Medicare annual wellness visits are now a covered benefit for seniors. This session focuses on meeting CMS requirements and the process used to identify the screening tools. Resources are provided that will assist in developing these visits in your organization.
Learning Objectives:
Identify the most appropriate and cost-effective care provider to perform the Medicare Wellness Visit.
List the criticial elements for the Medicare Annual Wellness Visit as established by the Centers for Medicare and Medicaid Services (CMS).
Select tools that contain the critical elements necessary to perform the Annual Wellness Visit.
The Role of the RN Care Manager in the Geriatric Assessment Clinic: Our Unique Medical Home Model for the Cognitively Impaired: This session presents a discussion on a multidisciplinary clinic which provides care and support for geriatric patients with cognitive impairment. Utilizing a medical home model, the presenters focus on 1) maintaining & improving the functional status of patients, 2) providing psychosocial support for the patient & caregivers, and 3) ensuring patient safety.
Learning Objectives:
Define the role of the RN Care Manager as coordinator/facilitator in the medical home model.
Identify the medical and emotional needs of the dementia patient and their families.
