In May 2018, AAACN convened 41 national nurse and health care leaders for a Care Coordination and Transition Management (CCTM) Invitational Summit. The purpose of the summit was to create a strategic, collaborative agenda aimed at defining the role of the registered nurse in CCTM across the care continuum.
