Addressing therapeutic inertia in diabetes care, this session explores the positive impact of a collaborative partnership between primary care nurses and diabetes educators (DE) on improving the quality of care for patients diagnosed with diabetes mellitus and elevated Hgb A1c (A1c) levels.
