Objectives:
1. Discuss processes of developing and using a logic model to capture impact and value for CCTM in ambulatory care done by RNs. 
2. Delineate ways that the RN in CCTM logic model can be used to provide a foundation for development of standardized documentation in EHRs as a basis for regulatory benchmarking and reporting, IP team staffing, models, and contribution to patient and organization outcomes. 
3. Discuss use of RN in CCTM logic model in developing a business case for investment in RNs doing CCTM in ambulatory care.
