Transitions in Care: An Opportunity to Highlight the Value of Nursing

Nursing has a unique opportunity to leverage its value by demonstrating positive cost-effective outcomes for patients. This session explores the cultural changes occurring in primary care as options for care delivery. Transitional care from the inpatient to outpatient setting is presented as an example.
