Transparency in Data: The Design of an Ambulatory Care Harm Grid

This session describes the design and implementation of an ambulatory care harm grid to build a culture of safety. This innovative solution helps to benchmark quality and patient safety data to ensure transparency, raise awareness of opportunities for quality and patient safety initiatives, and recognize best practices.
