This panel presentation shares their approaches to value-based care through navigation, care management and population health. Hallie Bleau, Vice President of Care Management from Northwell Health, discusses how a large integrated delivery network approached value-based care, navigation, care management. Katharine Green from the clinical quality and data management team at Dartmouth-Hitchcock Putnam Medical Group at Southwestern Vermont Health Care discusses a path to sustainability for population health in the medical practice setting. Jennifer Edwards, RN Clinical Services Director for Banner Health, lays the groundwork for building a robust chronic and complex care navigation program in an academic medical center and medical group.
