Risky Business: One Health Care System's Model of Risk Stratification

The Sanford Health risk stratification model is a two-step process that helps patients manage their care. It's based on a patient’s utilization and chronic conditions and uses evidence-based screening tools. This session discusses risk stratification and how this multidisciplinary model of care helps create successful outcomes and transitions.
