242 - Advanced Illness Care: A Patient-Centered Approach to End-of-Life Care

Using scenario-based examples, this presentation describes how advanced illness care was implemented in a large integrated health care system using ambulatory care RN care coordinators to identify and manage patients in advanced stages of illness.
