Colette Carver, APRN-BC-ADM, FNP, is a Family Nurse Practitioner for a large healthcare system. Her current focus is the implementation of the Patient-Centered Medical Home Model for the organization. After receiving her masters in nursing from Duke University, she joined the Duke Family Medicine staff, where she participated in such performance improvement projects as Advanced Access Scheduling, Office Redesign Strategies, and the Community Care of North Carolina project. She has been actively involved in health education as a former instructor for the University of Pennsylvania in Philadelphia, as a Diabetes Educator, and as a preceptor for nurse practitioner students. She has played a leading role in obtaining NCQA Patient-Centered Medical Home Certification for over 17 primary care practices, all with a Level 3 designation. She continues to provide foundational work in promoting the role of nurses in care coordination among ambulatory practices.
