Learn how a team of RN quality coordinators initiated a population health improvement program to standardize care gap closure and provide excellent patient care in primary care practice. Focus includes the development of the initiative and the efforts to create a multidisciplinary team-based approach to develop best practice quality improvement (QI) workflows.
