Proactive Office Encounter - One Step Ahead: Optimal Integrated Care for Every Patient Encounter

Improving the quality of preventive and chronic care involves all members of the ambulatory health care team. This presentation focuses on how the Southern California Permanente Medical Groups implemented the Proactive Office Encounter initiative, a collaborative effort that utilized all members of the health care team to engage patients. The initiative integrated processes, tools, and workflows to proactively address preventive and chronic care needs before, during, and after each encounter.
