Targeted Approach to Achieving the Quadruple Aim

This session describes how a community-based case management program of advanced practice nurses and social workers addresses the needs of vulnerable adults at high risk for readmission due to complex medical conditions. The ways that the program helps clients navigate their care are also explored, as well as the program's impact on the entire health care system and community.
