221 - Ambulatory Care RN Care Coordination Competencies

This session examines the role of RN care coordination competencies. An overview of the work of AAACN expert panels on care coordination and transition management is used to facilitate discussion on the dimensions of care and the importance of a patient-centered model that harmonizes the role of the RN in care coordination with the interprofessional care team.
