Staying on Target: Impact of Behavioral-Based Interviewing by a Health Care Coach in Patient- Centered Medical Home for Management of Chronic Disease: This presentation describes the current state of chronic disease management and its impact on health care in the U.S. The theory behind behavioral-based interviewing techniques is reviewed, along with a discussion on how one health care coach was able to empower patients for self-care management and improve outcomes in a PCMH model.

Motivational Interviewing: Improving Patient Outcomes: This presentation is designed to acquaint nurses with the literature support, theory, and techniques for promoting change in health care behaviors. Three core skills are shared to showcase the partnership between nurse and patient in facilitating positive health outcomes and a research-tested model of motivational interviewing is discussed.
